Posting a null: black seed oil did nothing for me
Black seed oil was the first thing I tried that had actual trials behind it rather than testimonials. Didn't do much for me personally. Posting the null because this place should have nulls in it.

Why is photobiomodulation absent from our wound-care protocols?
Photobiomodulation keeps showing up in wound-care literature and almost never in our protocols. I'd like to know why the gap is that wide — I suspect reimbursement, not evidence.

Omega-3 and cardiovascular risk: dose and form decide the outcome
The omega-3 cardiovascular literature is a lesson in how dose and form decide outcomes. Low-dose mixed EPA/DHA trials mostly disappoint. High-dose EPA ethyl ester tells a different story. Reporting those together as "fish oil doesn't work" is bad reading.

Meta-analyses that pool incompatible doses produce confident nulls about nothing in particular.

On quercetin: strong in vitro story, modest human data, enormous online confidence. That gap is where credibility goes to die.

I still use it, and I tell patients exactly where the evidence stops. Saying "this is plausible and under-tested" costs nothing and keeps you honest.

What the glyphosate microbiome literature actually shows
The glyphosate–microbiome argument rests on the shikimate pathway: plants and many bacteria have it, humans don't. That makes the mechanism plausible and the dose question decisive.

Where I think both sides overreach: "mechanism exists" is not "effect at environmental exposure," and "no effect at environmental exposure" is not established either. This is a genuinely open question and it is being argued as if it were closed.

A1c 9.1 to 5.4 in fourteen months. Time-restricted eating, walking after meals, and dropping liquid sugar. My endocrinologist was skeptical, then surprised, and now refers people to the same protocol.

I'm one person — n=1, no control, every confounder you can name. The trials below are the reason I tried it at all. #fasting

Night shift and magnesium: what finally helped
Night shift observation, offered as observation and nothing more: the two cheapest things that seem to help patients sleep on our unit are dimming the lights at 9pm and magnesium with the evening meds. One of those is free.

Labelling this personal experience, not evidence — though the magnesium literature isn't empty.

Berberine and metabolic markers: what I see in clinic
Berberine comes up in my clinic constantly, usually as "nature's metformin." The head-to-head data is more interesting than that slogan: comparable glycemic effects in several trials, different side-effect profile, meaningful GI intolerance, and real drug-interaction concerns via CYP3A4.

I'll say the unfashionable thing in both directions: it is not a herb to wave off, and it is not a reason to stop a working prescription without supervision.

Melatonin is not only a sleep hormone
Melatonin as a sleep hormone is the least interesting thing about melatonin. The mitochondrial pool is produced locally, at concentrations well above serum, and behaves like an antioxidant system rather than a circadian signal.

Methodological caution: a lot of this work is in vitro at concentrations no oral dose reaches. I'm flagging my own citation as preclinical for exactly that reason. #epistemics

Selenium and TPO antibodies: the meta-analysis I keep sending patients
Clinical pattern I keep seeing and can't fully explain: patients with autoimmune presentations tend to arrive with 25(OH)D in the teens and twenties. Correlation is cheap — low D is also a consequence of chronic inflammation, not only a cause.

What I'd want before claiming causation: randomized supplementation with disease-activity endpoints, not just serum normalization. Some of that exists now. It is more mixed than either camp admits.
